Definition:
- Flexibility on the amount of months of service delivery; no minimum or maximum
- Minimum of 60% direct service delivery
- Typically averages 1 session per week but will vary dependant upon needs for resource and referral integration
- Family Centered Evaluation available per need
- 24/7 on call availability
- User friendly hours of service delivery (nights/weekends)
- Provision of some limited treatment services after the assessment is completed
- A treatment team meeting is planned for at the end of the first thirty days and as needed on an ongoing basis. The team will consist of all agency representatives and others who are involved in the child and family’s life. The team members, including the family, will decide how often the team meets.
- Work with the local Child and Family treatment team to hold all agencies accountable for services the family may need.
- All stakeholders define the family as the treatment team leader. Family is involved in all meetings. They determine the goals for change.
- Utilizes a family systems approach to identify the strengths and challenges for each family.
- Helps family identify and establish support systems (both informal and formal)
- Services occur in the home, school, or any community setting where a need has been identified.
- Services include planning and coordination as well as integration of the multiple services being provided. Services will also include linking the family to evaluation, treatment, educational, vocational, residential, health, financial, social and any other needed services.
- Targeted goals may be utilized by some contracts I.E. anger management, social skills, stress management, decision making skills, anxiety management, etc.
- With some contracts neither the family nor IFCS determines length of involvement – external system / factors rule.
